Mittagong Public School Student Health Condition Requirements

At Mittagong Public School we aim to support the health needs of all our students. If your child has a need for health support, or medication, at school please call the front office 48 711 020 or speak to your child’s teacher.
Prescribed Medications
If your child needs to take medication during school hours please provide the following:
- Prescribed Medication Form (click to access form)
- Medication in original packaging, (preferably a Webster pack), clearly labelled with your child's name, dosage & time.
Note: The Education Dept does not allow staff to administer ANY medication to a student unless it has been provided by a parent/carer along with the signed Prescribed Medication Form.
To enable Mittagong Public School to support your child's health needs at school we request that you complete the following form if your child has a health condition.

Mittagong Public School also requires that you provide extra documentation for the following health conditions:
Asthma
Please provide the school with:
- An up to date, signed ‘Asthma Action Plan' from your child's doctor.
- Request for student to carry own asthma reliever/puffer form (click to access form)
- For younger students please contact the Office if you would like their puffer and spacer stored in sick bay.
Note: Asthmatic students should carry their 'puffer' on them, a spacer, and their Asthma Action Plan at all times.
Anaphylaxis
Please provide the school with:
- An up to date 'ASCIA Action Plan' (anaplan) signed from your child's doctor
- An EpiPen/Anapen to be stored in sick bay.
- A request to carry EpiPen/Anapen form (click to access form)
Note: Anaphylaxis students should carry their own EpiPen/Anapen on them at all times with a copy of their ASCIA Action Plan for Anaphylaxis.
Diabetes
Please provide the school with:
An up to date 'Diabetes Action Plan' signed from your child's doctor/Diabetes support team.
Note: Diabetic students should carry a hypo kit on them, water and their Diabetes Action Plan at all times.

- the behaviour code for students, which outlines the standards of behaviour expected in all NSW public schools
- anti-racism education
- anti-bullying programs
- conflict resolution and mediation training
- peer support
- road safety education
- the Healthy School Canteen Strategy.

Like all NSW public schools, we promote the healthy development of students through:

- providing first aid and temporary care of students who become unwell or who have an accident at school.

At our school, we use Positive Behaviour for Learning – a whole-school approach for creating a positive, safe and supportive school climate where students can learn and develop. Our whole school community works together to establish expected behaviours and teach them to all students.
